Key Responsibilities
• Effective time management, caseload management, and problem-solving skills.
• Demonstrated experience in implementing clinical skills to plan, prescribe, implement and evaluate individualised, evidence-based physiotherapy programs in partnership with the clients and their representatives.
• Conduct comprehensive assessments of patients’ physical conditions and functional abilities.
• Develop and implement personalised treatment plans for musculoskeletal issues.
• Provide community physiotherapy services through home visits, ensuring convenience and accessibility for clients.
• Educate patients and their families on injury prevention, falls prevention, safe and effective mobility strategies, home safety assessments and lifestyle modifications.
• Collaborate with other healthcare professionals to ensure integrated care.
• Maintain accurate and up-to-date patient records and documentation.
• Participate in clinical review meetings and case conferences as required.
• Assessing a client’s physical condition to diagnose musculoskeletal conditions and other related illnesses in alignment with acute care practitioner status.
• Creating a tailored treatment plan to address client’s identified impairments, considering their age, fitness levels and any other medical conditions.
• Communication with patients, other physiotherapists, referring practitioners and surgeons whilst upholding the values of South Burnett Physiotherapy.
• Provide a high level of customer care and service skills whilst liaising with patients, families, doctors, surgeons, and other health providers.
Qualifications
• Bachelor’s or Master’s degree in Physiotherapy and relevant registration with AHPRA
• Experience in providing community-based physiotherapy to elderly individuals and those with physical and cognitive disabilities.
• Proven experience in musculoskeletal physiotherapy highly desirable.
• Excellent communication and interpersonal skills.
• Ability to work independently and also as part of a collaborative team.
• Valid driver’s license and access to a reliable vehicle for home visits.
