Care finders connect in Toowoomba

In August, the PHN team hosted a meeting with care finders from across the region to share information and reflect on the great work being done to assist older people navigate the aged care system.

Care finders support vulnerable older people who would not be able to arrange services without intensive support and do not have a family member or friends who can help. Since the program launched in January 2023, care finders have supported 3,333 people to navigate aged care system to ensure they have the support they need.

These services are provided at no cost as they are fully funded through Darling Downs and West Moreton PHN.

In our region, care finders come from three commissioned organisations; Aged and Disability Advocacy Australia (ADA), Footprints Community Limited, STAR Community Services.

At the meeting the care finders discussed challenges, shared solutions and discussed positive outcomes.
