This program is designed to improve accessibility of care for priority groups within the Somerset and Southern Downs regions, in order to:
- increase the self-efficacy in the management of peoples own chronic conditions;
- decrease demand for treatment services to manage chronic conditions and functional decline;
- and decrease potentially preventable hospitalisations relating to chronic conditions.
This service requires a GP referral.
