From 28 October to 9 December 2024 , we want to TALK ABOUT preventative health and healthcare for chronic conditions.

This survey is open to anyone living in the Darling Downs and West Moreton PHN region. There is an optional extended part of the survey for people with living experience of one or more chronic conditions.

The TALK ABOUT campaign has been asking community members and health professionals about their experiences with health care since January 2020. In that time, the Darling Downs and West Moreton PHN have collected valuable feedback about various topics.
We continue to connect with people in our region to TALK ABOUT topics that matter most to them and their health, previous topics and WHAT WE HEARD documents are available below.
